Ventral Tegmental Area Disconnection Contributes Two Years Early to Correctly Classify Patients Converted to Alzheimer’s Disease: Implications for Treatment

Background: Recent cross-sectional studies highlighted the loss of dopaminergic neurons in ventral tegmental area (VTA) as an early pathophysiological event Alzheimer’s disease (AD). Objective: In this study, we longitudinally investigated by resting-state fMRI (rs-fMRI) a cohort patients with mild cognitive impairment (MCI) due to AD evaluate impact VTA disconnection predicting conversion AD. Methods: A 35 MCI were recruited and followed-up for 24 months. They underwent evaluation rs-fMRI assess connectivity at baseline follow-up. Results: At 24-month follow-up, 16 out converted Although converters non-converters did not differ demographic behavioral characteristics baseline, first group showed significant reduction VTA-driven posterior cingulate precentral cortex. This pattern additional MCI-Converters compared remained substantially unchanged Conclusion: study reinforces hypothesis contribution dysfunction evolution targeting default-mode network.
